New clients, please read and complete the information below to help us make your visit as helpful, enjoyable, and efficient as possible
Before your first visit to
The Anxiety Treatment Center of Greenwich
Please download, print, and complete the appropriate New Patient Form — the Child New Patient Form if the patient is under 18, or the Adult New Patient Form if the patient is 18 or older. In addition, all patients must fill out the Consent for Treatment Form, Payment Information Form, Communication Policy Form, and Release of Information Form. Please bring all completed forms with you to your initial appointment.

*While we do not bill insurance directly, we encourage you to explore your out-of-network benefits. Our services are generally reimbursed by health savings accounts.
